In short, while shortened luteal phases correlate with fertility struggles, science cannot confirm LPD is the direct root cause; it may simply be a secondary side effect of other underlying conditions.

LPD Treatments: Limited Supporting Clinical Evidence

Given these unresolved uncertainties, ASRM takes a restrained stance on LPD interventions.

The situation is different during the IVF cycle. Luteal phase support (i.e., supplementing progesterone after transplantation) is a routine procedure in IVF treatment. However, this therapy addresses suppressed corpus luteum function triggered by ovarian stimulation drugs, not targeted treatment for LPD itself.

The ovulation-inducing drugs used in the IVF cycle inhibit the patient's own luteal function, so external luteal support is required regardless of whether the patient has luteal phase defects.

ASRM’s 2026 guidelines do not recommend routine LPD screening or intervention for patients pursuing natural conception.
